Position Summary:
The Registered Nurse is responsible for the clinical care of assigned home care patients which may include but not be limited to case management of the case, specific nursing tasks, patient assessment, patient/family education, and coordination with the multidisciplinary team including RN, LPN, PT, PTA, OT, COTA, SLP, MSW and Home Health Aides.
The RN is responsible to report changes in condition to the physician and to the Clinical Coordinator as appropriate.
The RN reports to the Clinical Coordinator.
Locations: Washington ( Tacoma, Federal Way, Seattle, Everett, Marysville )
Education and Experience:
Graduate of an approved school of professional nursing with a current license in the state in which you are practicing
Minimum of 1 year experience as a RN
Home Health experience preferred
Must have CPR certification
Salary: Depends upon experience and location
What is Home Health? Nursing and Therapy team members visit patients in their homes on an intermittent basis according to doctor’s orders. Our skilled professionals work one on one with patients, caregivers and family members. Home Health allows health care professionals to utilize all of their accumulated knowledge and skills to help individual patients reach their goals as part of a multi-disciplinary care team. Staff perform 5-7 patient visits daily on average.
